New enemies, including the one-eyed master swordsman Yagyu Jubei, appear, and defeating them allows players to acquire new equipment and Soul Cores to increase their strength. In the high-difficulty mode 'Path of Enlightenment' (3rd playthrough), some enemies in the field are randomly granted the 'Curse' buff, allowing players to enjoy high-risk, high-reward gameplay. Additionally, elements that enable more powerful builds, such as higher-tier gear bonuses and powerful rare special effects with higher values, are being added. It also includes hardcore content where players can repeatedly enjoy battles with vast patterns and refine scroll effects through the 'Scroll of the Hundred Demons'.

To use this content, the separately sold base game 'Nioh 3' is required, and each piece of content can be played after progressing through the game to a certain level. This DLC is also included in the 'Nioh 3 Season Pass 3' and 'Nioh 3 Digital Deluxe Edition'. The release date for the second DLC, 'Amakusa Rebellion', is currently undecided.
